Job description

Description

This is a full time, direct care position. Applicant must be a be at least 18 years of age, have a high school diploma (or equivalent), and have two (2) years paid experience performing the following types of functions (listed below), with at least one year of paid experience working directly with behavioral/mental health clients.  The primary responsibilities of the Extender are to:   

  • Perform general outreach, engagement, and follow-up with members 
  • Coordinate services/appointments 
  • Engage clients in health promotion activities 
  • Share information with Care Manager on the member's circumstances 
  • Make and track referrals 
  • Participate in case conferences 
  • Support the care manager in assessing and addressing unmet health-related resource needs.
