Job description

Overview

Our Lady of Consolation is a proud member of Catholic Health and a recognized leader in providing exceptional quality driven care. We offer Long Term, Post-Acute and Skilled Nursing services. Our Lady of Consolation is located on the beautiful Great South Bay and is comprised of a 345-bed nursing and rehabilitative care center. Our nurses, physicians and support staff are devoted to providing patient driven health care in an environment of compassion to our patients, their families and our community.

The Certified Therapeutic Recreation Specialist (CTRS) is responsible for assessing residents' recreational, social, emotional, cognitive, and physical needs and developing therapeutic recreation programs that enhance quality of life, promote independence, and support psychosocial well-being within the nursing home setting. The CTRS works collaboratively with interdisciplinary team members to provide meaningful activities and therapeutic interventions for residents.

Responsibilities

Key Responsibilities:

  • Work with Memory Care/Alzheimer's residents.
  • Attend care plan meetings with team. Develop care plans, goals and approaches.
  • Complete EMR and MDS progress notes and care plan documentation.
  • Assess patients/residents for leisure needs.
  • Develop and maintain calendar for memory care unit.
  • Implement special events on the unit and in the auditorium, such as crafts and creating customs
  • Plan community outings for long term residents. Reserve location. Prepare cost estimate for planned trips and activities.
  • Complete daily statistics of program attendance.
  • Transport residents throughout the facility to attend therapeutic programs
  • Ability to use software programs including but not limited to, Activity Connection IN2L, Apple Music, Microsoft Word, Excel, Canva, google slides and social media.
  • Assist with rehab assessments on weekends
  • Assist with music during Catholic mass on Sundays.

Requirements:

  • Current certification as a Certified Therapeutic Recreation Specialist required.
  • Bachelor's degree in Therapeutic Recreation, Recreation Therapy or related field.
  • Must be able to work required schedule, Sunday- Thursday 8:30am-4:40pm.
  • Must be able to work rotating holidays.
