What You Will Do
  • Serve as a key member of the Interdisciplinary Team (IDT), providing holistic, patient- and family-centered hospice care
  • Complete comprehensive nursing assessments, admissions, and ongoing reassessments in accordance with hospice regulations and timelines
  • Develop, implement, and continuously evaluate individualized plans of care in collaboration with the IDT and attending physicians
  • Provide skilled nursing care, including symptom management, pain control, wound care, IV therapy, and physical assessments
  • Educate patients and families on disease progression, medications, pain and symptom management, and routine caregiving tasks
  • Provide emotional support and compassionate guidance to patients and families throughout the hospice journey
  • Fulfill on-call responsibilities as assigned and assist with admissions or coverage during staffing needs

What We Are Looking For
  • Currently licensed as a Registered Nurse in the Commonwealth of Virginia
  • Minimum of two (2) years of nursing experience
    • Hospice or home health experience preferred
  • Strong clinical judgment, critical thinking, and decision-making skills
  • Excellent communication skills with the ability to educate, support, and collaborate effectively
  • Proficiency with EMR documentation and computer-based systems
  • Valid driver's license, reliable transportation, and proof of automobile insurance

Schedule & Benefits
Schedule: Flexible hours around a core schedule (Monday-Friday approx. 8:30 AM-5:00 PM)
Location: New River Valley Service Area
Benefits: 21 days of PTO in your first year, $.65 per mile mileage reimbursement, company paid life insurance and long-term disability insurance, medical through an ICHRA model offering multiple plan choices, and 401(k) with company match (100% match on the first 3%, 50% match on the next 1%)
Compensation: Starting Salary of $75,000/year
