Job description

Case Manager / Genesee Case Management/ Full-time (40hrs.)/ Days/ Flint MI

This position does REQUIRE working in the community and performing home visits.
The Case Manager primary responsibilities include: Working in the community and performing home visits. Coordinate all services necessary to maximize consumer recovery and independence for individuals with mental illness. In a residential or clinical setting, provide clinical and program consultation to direct care staff. Ensure case management compliance with agency standards and the practice model. Assist consumer to improve quality of life and maintain the highest possible ability to recover and function within the community. Implement strategies for obtaining services and supports that are goal-oriented and individualized
What You'll Do...
  1. Work in the community performing home visits.
  2. Ensure consumer involvement in the Person-Centered Planning Process (PCP).
  3. Facilitate the implementation of Individual Plan of Service.
  4. Perform comprehensive, ongoing psychosocial assessments.
  5. Participate in legal proceedings including petitioning and testifying in clinical emergencies.
  6. Provide community-based services.
  7. Facilitate the coordination of consumer intakes, discharges, and transfers.
  8. Communicate with all service providers in Treatment Plan.
  9. Facilitate hospitalizations, attend hospital meetings and coordinate with hospital social workers.
  10. Coordinate and monitor activities such as a job, school, volunteerism, and family caretaking.
  11. Provide referral to appropriate community resources.

  • Prior case management experience in a mental health or rehabilitation setting preferred.
