Job Description:

The Social Services Supervisor is responsible for assisting residents to manage their everyday emotional, mental, and psychosocial needs:

  • Advocate for resident needs and rights
  • Maintains contact with family, in consultation with the resident, to report on changes in health, goals, discharge planning
  • Makes referrals and obtain services from outside entities, e.g., talking books, absentee ballots, etc.
  • Assists residents with financial and legal matters, e.g., Social Security, VA, referrals to funeral homes for preplanning arrangements
  • Builds relationships between residents and staff and teaches staff how to understand and support resident's individual needs.
  • Participates in Interdisciplinary Care Plan Conferences and develop plan of care to ensure that special social service needs are met
  • Assists in resolution of grievances as voiced by resident, family, responsible party, or ombudsman
  • Completes Social Service Assessment Discharge Planning
  • Documents all identified social service problems with appropriate approaches and time measurable goals
  • Manage other Social Services staff as required

Requirements:

  • Bachelor's in social work or human services. Masters preferred
  • BSW is required
  • One year of supervised social work experience in a healthcare setting
