Position Overview

Peak Performance Physical Therapy is seeking a dedicated and passionate Physical Therapist (PT) to join our team at our rehabilitation center in Port Charlotte, Florida. This full-time position requires an individual who can provide high-quality, patient-focused care to help our clients achieve their rehabilitation goals. The successful candidate will work closely with patients to develop and implement individualized treatment plans in alignment with their medical needs and personal goals.

Key Responsibilities
  • Direct physical therapy assessments and treatments aimed at preventing disability, reducing pain, enhancing muscle function, and maximizing patient independence.
  • Collaborate with physical therapy assistants, ensuring the appropriate execution of care plans, and providing oversight as necessary.
  • Conduct comprehensive evaluations of clients' functional abilities through diagnostic tests related to muscles, nerves, joints, and overall physical capabilities.
  • Assess client home environments to provide recommendations that foster independence and safety during rehabilitation.
  • Implement physician-prescribed physical therapy services, which may include therapeutic exercises, manual therapy techniques, and application of physical agents for rehabilitation.
  • Monitor and document changes in client conditions, adjusting care plans based on observed responses to therapy.
  • Engage actively with interdisciplinary teams during case conferences to ensure holistic treatment approaches and to share insights pertinent to client care plans.
  • Prepare clinical documentation, including evaluations, care plans, progress notes, and discharge summaries, ensuring compliance with healthcare regulations.
  • Guide and instruct clients and families in the proper use of therapeutic devices and the overall physical therapy program.
  • Complete functional assessments and collaborate on community resource utilization to support patient recovery.
Patient Population / Client Focus

This role focuses on a diverse patient population ranging from post-surgical rehabilitation patients to individuals recovering from acute and chronic conditions. You will have the opportunity to innovate and tailor therapy approaches for clients with various needs, ensuring each patient receives care that is both individualized and impactful.

Qualifications
  • A Doctorate of Physical Therapy (DPT) from an accredited institution.
  • State licensure as a Physical Therapist, maintaining good standing with regulatory bodies.
  • Strong problem-solving skills and the ability to make informed decisions in fast-paced situations.
  • An understanding of current best practices and therapeutic techniques in physical therapy.
  • Ability to work collaboratively within a multidisciplinary team and communicate effectively with patients and their families.
Preferred Experience
  • At least one year of experience in physical therapy, preferably in an acute care or home health setting.
  • Experience in developing and modifying treatment plans based on patient evaluations and outcomes.

The salary for this position is competitive, with an annual salary range of USD 100,000 to 120,000.
