This physical therapist position is onsite, specifically tailored for home care settings. You will travel to patients' residences, providing therapy in familiar environments that facilitate comfort and progress. You will collaborate closely with multidisciplinary teams, including physicians and nursing staff, to ensure comprehensive patient care.

• Assess and evaluate patients’ conditions, identify impairments, and establish tailored treatment plans aligned with physicians' orders.
• Implement therapeutic interventions aimed at improving mobility, strength, and functionality, closely monitoring patient progress.
• Document all patient care activities meticulously, maintaining detailed records of evaluations, treatment plans, and modify strategies based on patient feedback and outcomes.
• Supervise the activities of Physical Therapist Assistants (PTAs) and support personnel, ensuring they provide safe and effective patient care.
• Engage in regular collaboration with healthcare professionals such as physicians, occupational therapists, and speech-language pathologists to coordinate comprehensive treatment plans.
• Educate patients and their families about their conditions and involve them in decision-making processes regarding their care paths.
• Contribute to maintaining high productivity levels within the clinic while providing constructive suggestions and problem-solving assistance to management.

As a physical therapist at Peak Performance, you will serve a diverse patient population, including individuals recovering from surgery, those with chronic pain conditions, and elderly patients requiring rehabilitation services. Your role will involve understanding their unique needs to deliver personalized care that fosters their recovery and enhances their functional abilities.

• Bachelor of Science degree in Physical Therapy from an accredited university.
• Current licensure as a Physical Therapist in the state of Virginia.
• Minimum of two years of experience in a physical therapy setting is required.
• Strong verbal and written communication skills essential for effective interaction with patients and multidisciplinary teams.
• Proficient in using electronic medical record systems for documentation and patient management.

Experience in home health settings and working with geriatric populations are preferred. This full-time position offers a competitive annual salary ranging from $80,000 to $100,000, commensurate with experience.
