Skylake Post Acute is Hiring an Experienced Full-Time MDS Coordinator - RN/LPN!
Schedule: Full-Time

As one of the largest skilled nursing facilities (SNF) in Colorado, we pride ourselves on being the top choice for rehabilitation and 24-hour skilled nursing care.


What to Expect:

  • Collects MDS documents within mandated, established and expected time frames.

  • Compares data entered against the original MDS document; makes corrections as needed.

  • Files the MDS documents in residents' charts.

  • Maintains confidentiality of all patient care information.
