As a valued member of our interdisciplinary team, the MDS Coordinator is responsible for ensuring the highest quality care for our residents. This is a full-time opportunity to work with a talented team of professionals who are dedicated to providing exceptional care and service to our patients.

About the Role:

As an MDS Coordinator at Aristos Rehabilitation and Nursing Center, you will be responsible for coordinating the Minimum Data Set (MDS) process, conducting resident assessments, and ensuring that all necessary information is accurately recorded and submitted to regulatory agencies. Your strong organizational and communication skills will be essential in ensuring that our interdisciplinary team has the necessary information to provide individualized care plans for our residents.

Responsibilities:

• Coordinate the MDS process, including conducting resident assessments, reviewing assessments, and submitting MDS data to regulatory agencies
• Ensure accurate and timely completion of MDS assessments, care plans, and progress notes
• Collaborate with the interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each resident
• Maintain accurate and up-to-date records and documentation
• Participate in quality improvement initiatives to ensure the highest level of care for our residents
• Provide education and training to residents, families, and staff on MDS processes and regulations

Qualifications:
