MDS Nurse - LVN or RN

Gateway Post Acuteis seeking a knowledgeable and organizedMDS Nurse - Licensed Vocational Nurse (LVN) or Registered Nurse (RN)to join our interdisciplinary team. This full-time position is ideal for a detail-oriented nurse who is committed to accurate resident assessments, regulatory compliance, and high-quality care.

Compensation
  • LVN:$72,000-$78,000 annually
  • RN:$82,000-$90,000 annually
  • Compensation is based on experience and qualifications.
Responsibilities
  • Coordinate and complete accurate MDS assessments within required timeframes.
  • Collect, review, and verify resident clinical information.
  • Ensure MDS documentation complies with federal, state, and facility requirements.
  • Monitor assessment schedules and maintain timely completion of all required documentation.
  • Collaborate with nursing, therapy, social services, dietary, and other interdisciplinary team members.
  • Participate in care plan meetings and assist with the development of individualized resident care plans.
  • Review medical records to ensure documentation supports resident needs, services, and reimbursement.
