FULL TIME MDS COORDINATOR

MUST BE REGISTERED NURSE

MUST HAVE PRIOR EXPERIENCE AS MDS COORDINATOR

Oversee transmission of MDS and billing procedures related to MDS. Must possess significant experience as MDS Coordinator with thorough knowledge of all aspects.

Duties :

  • Completing accurate assessments, MDS and care plan as assigned

  • Creating and distributing monthly care plan in a timely fashion

  • Monitoring and auditing clinical records

  • Informing DON of persistent issues related to non-compliant documentation

Requirements :

  • Must have prior experience with MDS 3.0 AND Medicaid process

  • Valid NYS Registered Nurse (RN) License

  • Long Term Care Experienced Required

  • Excellent Communication skills

  • Knowledgeable in PDPM
