Position Overview:

The MDS Coordinator assumes full time administrative authority, responsibility and accountability to coordinate the completion of the required minimum data set and with other members of the inter-disciplinary team, develops and implements a plan of care that meets the individual needs of each resident.

  • Oversee the completion of MDS assessments across multiple facilities

  • Ensure compliance with federal and state regulations regarding MDS documentation

  • Provide training and support to facility staff on MDS processes

  • Monitor and analyze MDS data for quality assurance

  • Collaborate with interdisciplinary teams to develop and implement care plans

  • Facilitate regular meetings to discuss MDS updates and best practices

  • Respond to inquiries and resolve issues related to MDS documentation


Qualifications:

  • Bachelor's or Associate's degree in Nursing

  • LPN or RN license in the state of Tennessee

  • A minimum of 2 years of experience in skilled nursing facility or healthcare coordination

  • Strong knowledge of the MDS process and related regulations

  • Excellent communication and interpersonal skills

  • Proficiency in data analysis and reporting

  • Ability to work independently and manage multiple projects simultaneously

  • Certification in MDS (such as MDS Coordinator) is preferred

  • CMI (Case Mix Index) experience is a Must!
