We are seeking a Hospice QA Lead based in our San Francisco office, focused on chart review, documentation accuracy, and regulatory compliance. In this role, you will help ensure that each patient's clinical needs, eligibility, and care are clearly and accurately reflected in the chart, while keeping the agency aligned with ACHC standards and Medicare requirements.
This is a detail-oriented, supportive role for someone who values both quality care and the documentation that stands behind it.
What You'll Do
โ€ข Perform daily chart audits for hospice patients
โ€ข Ensure compliance with Medicare hospice regulations and ACHC standards
โ€ข Review and validate certifications and recertifications, face-to-face encounters, hospice eligibility, and documentation of decline
โ€ข Review plans of care and visit documentation for accuracy and completeness
โ€ข Identify documentation gaps and support timely corrections
โ€ข Track trends in live discharges, revocations, length of stay, and documentation inconsistencies
โ€ข Partner with clinical leadership to ensure follow-through and accountability
โ€ข Maintain ongoing survey readiness through consistent chart review
โ€ข Provide clear, constructive feedback to clinicians to strengthen documentation
What You Bring
โ€ข Active RN license (California)
โ€ข Hospice experience required
โ€ข Strong understanding of hospice eligibility, recertification requirements, and documentation supporting terminal prognosis and decline
โ€ข Knowledge of Medicare hospice compliance standards
โ€ข Experience in QA, auditing, or clinical review preferred
โ€ข Strong attention to detail with a patient-centered mindset
โ€ข Ability to communicate clearly and work collaboratively with clinicians
Why This Role Matters
โ€ข Ensures patients are appropriately supported with hospice services
โ€ข Strengthens the accuracy and completeness of clinical documentation
โ€ข Maintains compliance and ongoing survey readiness
โ€ข Supports clinicians in delivering high-quality, well-documented care
Who This Role Is For
โ€ข You are detail-oriented but also see the bigger picture
โ€ข You take pride in doing things the right way
โ€ข You enjoy supporting clinicians and strengthening processes
โ€ข You believe that good documentation helps tell the full story of patient care
