Responsibilities
- Review assigned patient charts in the EMR and internal portal for DBQ/patient visit preparation.
- Identify and extract the date of incident and related timeline details required for DBQs.
- Locate and compile chart evidence that supports DBQ sections (using internal documentation already available in the EMR/portal).
- Assemble electronic DBQ review packets (folders and/or standardized summaries) for the psychiatrist, ensuring information is organized and easy to verify.
- Flag missing, unclear, or inconsistent information for follow-up by the clinical team.
- Maintain consistent naming conventions, folder structure, and documentation workflow standards across patients.
- Track review status (for example, complete/in progress/missing items) to support DBQ deadlines.
- Maintain strict confidentiality and follow EMR/portal access and HIPAA/privacy/security procedures.
Required qualifications
- Prior experience in chart review, medical records, intake support, or healthcare documentation support (preferred).
- Strong attention to detail and accuracy in extracting dates and locating entries.
- Ability to navigate an EMR and portal efficiently and follow structured workflows.
- Professional discretion and comfort handling sensitive mental health information.
- Reliable on-site attendance and time management to meet DBQ deadlines.
