Job description

Job Summary
The Community Outreach Coordinator builds and maintains strategic relationships with referral sources and community partners to increase awareness, access, and utilization of Wraparound and behavioral health services for youth ages 5-21. This role is responsible for driving referral growth, supporting community education, and strengthening partnerships across systems serving children and families throughout Louisiana's Coordinated System of Care (CSoC).
The Outreach Coordinator represents the agency professionally in the community and is accountable for tracking outreach efforts, referral activity, and engagement outcomes.
Essential Duties & Responsibilities
Community Engagement & Relationship Development
  • Develop and maintain strong referral relationships with schools, DCFS, juvenile justice, healthcare providers, and community organizations
  • Establish and expand partnerships to strengthen referral networks across the region
  • Serve as a primary liaison between the agency and community stakeholders
Outreach & Education
  • Schedule and deliver presentations on Wraparound services, eligibility, and referral pathways
  • Represent the organization at community events, resource fairs, and stakeholder meetings
  • Increase awareness of services through targeted outreach strategies
Referral Coordination & Collaboration
  • Collaborate with Intake and program teams to ensure smooth transition of referrals from inquiry to enrollment
  • Respond to community inquiries in a timely and professional manner
  • Conduct follow-up with referral sources to maintain engagement and strengthen partnerships
Data Tracking & Reporting
  • Track referrals, outreach activities, and engagement efforts
  • Monitor performance metrics including referral volume, conversion rates, and outreach effectiveness
  • Maintain accurate documentation of outreach efforts and referral sources
  • Prepare routine reports for leadership on outreach performance

Resource Development
  • Maintain outreach materials, partner contact lists, and community resource directories
Professional Conduct
  • Maintain professional boundaries and uphold ethical standards in all interactions
  • Manage schedule and outreach activities independently while meeting performance expectations
  • Referral-to-enrollment conversion rate
  • Number of active community partnerships
  • Outreach events/presentations conducted
  • Response time to inquiries
Minimum Qualifications
  • High school diploma required; Associate or Bachelor's degree preferred in Marketing, Communications, Human Services, Public Health, or related field
  • Minimum 2-3 years of experience in outreach, community engagement, healthcare, or social services
  • Strong communication, public speaking, and relationship-building skills
  • Ability to track and report data accurately
  • Valid driver's license and reliable transportation
Preferred Qualifications
  • Knowledge of Louisiana CSoC, Magellan, and Wraparound services
  • Experience working with youth and families
  • Familiarity with local community resources and referral systems

WAA Compliance Requirements
  • Be eligible for Medicaid/Medicare participation (no sanctions or exclusions)
  • Have no findings on the Louisiana State Adverse Action List
  • Complete TB testing
  • Complete AHA-recognized First Aid, CPR, and seizure training (medical professionals exempt)
  • Maintain all required Wraparound training and credentialing
