Job Summary:
Desire Home Care, Inc. is seeking a compassionate and experienced Licensed Vocational Field Nurse (LVN) to join our team as a per diem healthcare employee. This individual contributor role will be based in the East Riverside area and will involve providing health care services in the comfort of patients' homes. As an LVN Home Health Nurse, you will be responsible for providing individualized and comprehensive care to patients according to their specific health needs, while also adhering to all regulatory and ethical standards.
Compensation- Per Diem:
  • Routine Follow-Up Visit: $35-45
  • Wound VAC: $ 40-50

Responsibilities:
  • Provide skilled nursing care to patients in their homes, in accordance with established care plans and physician's orders
  • Monitor and assess patients' health status and report any changes or concerns to the supervising Registered Nurse
  • Administer medications and treatments as prescribed by the physician
  • Provide wound care, catheter care, and other nursing procedures as needed
  • Educate patients and their families on their conditions, medications, and self-care techniques
  • Collaborate with other healthcare professionals to coordinate and manage patient care
  • Maintain accurate and up-to-date patient records, including progress notes, medication administration records, and nursing assessments
  • Adhere to all regulatory and ethical standards, including HIPAA guidelines and infection control protocols
  • Communicate effectively with patients, families, and other team members to ensure the best possible outcomes for patients

Requirements: *New Graduates Welcomed*
  • Active and unrestricted LVN license in the state of California
  • Minimum of 1 year of experience as an LVN, preferably in a home health setting
  • Strong knowledge of nursing principles, practices, and procedures
  • Excellent clinical assessment and critical thinking skills
  • Ability to work independently and manage time effectively
  • Strong communication, interpersonal, and organizational skills
  • Must have a valid driver's license and reliable transportation
  • Must have current BLS certificate
  • Physical and TB clearance valid within the year
  • Ability to complete documentation via agency EMR system

Work Schedule: Require a minimum of 5-10 visits per week
