MDS coordinators have a wide range of responsibilities, which can include:
  • Providing support to patients and families during treatment by answering questions about medication, disease progression, and other concerns
  • Coordinating patient care across disciplines by working with physicians, nurses, social workers, occupational therapists, pharmacists, dieticians, and other healthcare professionals
  • Creating treatment plans based on each patient’s medical history and preferences to ensure the best possible outcomes
  • Coordinating patient care across multiple facilities to ensure that each patient receives the best possible care
  • Reviewing patients’ medical records to identify any issues that need to be addressed by the doctor or other members of the care team
  • Managing patient care plans, including setting up appointments with specialty care providers and ordering lab tests
  • Enforcing physician orders regarding medication and treatment plans
  • Communicating with patients and their families regarding treatment plans and progress
