As the Family Case Manager, you will play a pivotal role in individuals' lives, making a significant impact through:

  • Completes timely and accurate documentation in the Case Record, including but not limited to: referral intake packets and related intake information, referrals and related correspondences, entitlement documentation, and other areas as assigned.

  • Performs Catholic Charities Housing Office assessments of each new tenant/head of household upon their admission to housing and on an annual basis thereafter.

  • Develops and implements ISPโ€™s (every 6 months) monitors their effectiveness, guides family members in achieving positive results, reports outcomes, and makes changes as needed with an eye on positive results for the successful and healthy functioning of the family.

  • Maintains notes in the case record to document progress toward, and/or achievement of milestones and performance targets as developed in the ISP.

  • Conducts case conferences with the Director of Multi-Family Housing, CCHO staff, DSS workers, and other involved treatment providers as needed.

  • Cooperates in an ongoing supervisory process with the DSS, including but not limited to, attendance at regularly scheduled case management team meetings.

  • Attends meetings and advocates on behalf of the clients and the program. Escorts clients to appointments and meetings as needed.

Family Case Manager Minimum Salary: $ 20.00/hour

Family Case Manager Maximum Salary: $ 23.00/hour

Family Case Manager Schedule: Monday - Friday, 11:00am - 7:00pm (37.5 hours/week)

  • 1 - 3 years of experience working with a diverse population
  • Valid driverโ€™s license, proof of current car insurance and registration and access to a reliable, working vehicle
