Job description

ORGANIZATION OVERVIEW:
Face to Face Health and Counseling is a non-profit organization dedicated to advancing economic and health equity for youth ages 11-24. We do this work through a comprehensive approach that includes housing, mental health, medical, youth justice, education & employment, and community services. We take a strengths-based and trauma-informed approach to our work and center the needs of young people impacted by racism and other forms of oppression and marginalization. Founded in 1972, Face to Face serves more than 3,000 youth annually.
JOB SUMMARY:
The Registered Nurse (RN) provides outreach nursing services at the day shelter (SafeZone) and clinic-based nursing care at the Face to Face clinic. Working collaboratively with the Nurse Practitioner and interdisciplinary team, the RN delivers trauma-responsive, harm reduction-based care to youth experiencing homelessness while promoting access to healthcare and health education.
This position is primarily based at SafeZone, Face to Face's day shelter that provides a supportive environment for youth experiencing homelessness and connects youth to basic needs, resources, case management, and health services. The RN builds trusting relationships with youth, provides nursing assessments, health education, reproductive healthcare, first aid, and referrals, and may serve as the only healthcare professional onsite. The RN also provides regular coverage at the clinic and is cross-trained to support medical assistant workflows as needed.
This is a full time, 30 hours per week position scheduled Monday through Friday between 9:00am and 6:00pm. While based primarily at SafeZone, the RN will provide regular nursing coverage at the Arcade Clinic one day each week, with additional coverage as needed.
RESPONSIBILITIES:
General Clinical & Patient Care
  • Provides all services guided by the 9 evidence-based principles for helping youth overcome homelessness.
  • Build trusting relationships with youth by meeting them where they are and providing trauma-responsive, harm reduction-based nursing care.
  • Triage patient phone calls and walk-in youths.
  • Provide nursing assessments, health education, reproductive healthcare, pregnancy testing, STI screening, emergency contraception, first aid, and referrals as appropriate.
  • Complete nurse visits and document care in NextGen, the electronic health record.
  • Resolve patient problems and needs by utilizing team-based strategies.
  • Perform clinical support tasks as needed, including rooming patients, obtaining vitals, and assisting with blood draws or other procedures within scope (cross-functioning as a Medical Assistant when needed).
  • Assist providers with patient care needs, procedures, and clinical workflows.
  • Promote youth's independence by establishing patient care goals; teaching patient, friends, and family to understand condition, medications, and self-care skills; and answering questions.
  • Attend and actively participate in team meetings and case discussions to support coordinated care for youth.
  • Step in and help to "get the job done" when short-staffed, including assisting with tasks outside of primary duties such as essential front desk and medical assistant responsibilities.
  • Perform other duties as assigned.
  • Provide nursing support to young people and be available to SafeZone staff for consultation related to health needs.
  • Maintain readiness of emergency equipment and supplies, including AED checks, first aid inventory, and scheduling sharps container pickups.
  • Assist with staff emergency drills such as epi-pen trainings as directed by SafeZone leadership.
  • Step in to support clinic operations during staffing shortages as needed.
  • Provide direct nursing support for general medicine, reproductive health, and OB visits.
  • Prepare for and assist residents during general medicine and prenatal residency clinic days.
  • Assist residents and providers with patient follow-up, treatment, and post-appointment referrals.
  • Provide regular nursing coverage at the Arcade Clinic one day per week and additional coverage as needed.
Clinical Operations
  • Respond to medical records requests.
  • Maintain patient confidentiality and protect health information in accordance with HIPAA and organizational policies.
  • Assist in implementing the vaccine program including the Minnesota Vaccine for Children program and UUAV.
  • Assist with medication dispensary inventory and stocking of supplies.
  • Ensure a safe and clean working environment by complying with procedures, rules, and regulations.
  • Assure quality of care by adhering to therapeutic standards and standards of nursing practice.
  • Protect patients and employees by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations.
QUALIFICATIONS:
  • Associate, Diploma, or Bachelor's degree in nursing.
  • Current licensure to practice as a RN in Minnesota.
  • Demonstrated knowledge of the Minnesota Nurse Practice Act and the Scope of Ambulatory Care Nursing.
  • Current Basic Life Support for Healthcare Providers (BLS-HCP) course completion.
  • Experience in prenatal/OB nursing is beneficial, though not required.
  • Excellent computer skills and experience with electronic health records.
  • Experience working with adolescents and young adults preferred.
  • Self-motivated with the ability to proactively engage young people, identify needs, and work independently while collaborating effectively with the interdisciplinary team.
  • Demonstrated ability to work and communicate effectively with people of diverse ethnic, economic, racial and cultural backgrounds.
  • Experience in mental health and/or substance abuse is preferred, though not required.
  • Ability to work in a team-based environment with flexibility to support multiple clinical roles.

  • Exposure to communicable diseases, toxic substances, medicinal preparations, and other conditions common to a clinic environment.
  • Requires manual dexterity and coordination sufficient to operate a copier, telephone, scales, thermometer, and sphygmomanometer; to administer oral and IM medications.
  • Responsibilities may involve occasional lifting of equipment/supplies, usually less than 25 pounds.
  • Face to Face is subject to the government mandate requiring employee COVID vaccination. All employees will be required to get vaccinated.

BENEFITS:
The pay rate for this position is between $41 - $44.50 per hour based on qualifications.
