JOB SUMMARY
The Licensed Practical Nurse (LPN) performs nursing care under the direction and supervision of the respective department Director/Manager and/or Shift Supervisor. The LPN oversees the work of CNAs on the unit and directs emergency response when necessary. The LPN assists providers (Physician, nurse practitioner, consultant pharmacist) on the unit as requested and as the practicing licensed professional on the unit, ensures all documentation is timely and correct, all medications are dispensed as written, and that any/all errors are promptly reported and remediated.
ESSENTIAL JOB FUNCTIONS
  • Supervises, assigns, and monitors work performed by CNAs; makes the necessary number of rounds to ensure that quality nursing care is being provided; provides backup support as needed and takes immediate corrective action when necessary.
  • Provides training to clinical staff in accepted work methods; provides information to management regarding further training needs; provides input for nursing assistants' performance evaluations and discipline
  • Complies and enforces department and company policies and procedures including standards of conduct, attendance, dress, and safety
  • Assists with resolving staffing needs and emergency staffing problems when necessary
  • Maintains positive and professional relationships with residents, guests, and other Williamsburg Landing employees; monitors staff for professionalism and positive interactions
  • Assists with the planning and overall nursing care of the residents on the unit; monitors residents' medical records for accuracy and timeliness of information updates; assists respective clinical leader with chart audits and other pertinent information to include the 24 hour chart checks; documents and completes required tasks in electronic medical record
  • Performs professional nursing tasks and nursing procedures to include, but not limited to: medication administration, ordering prescriptions, performing treatments as ordered, etc.
  • Participates in shift hand offs with the previous and next shift's nursing staff on residents with significant changes and assures all shift responsibilities are completed
  • Keeps providers and clinical leaders informed of any significant changes in resident conditions, vital signs, and other developments as they occur; maintains vigilance to observe any unusual symptoms or signs of illness or skin breakdown and reports same to clinical leader/supervisor in charge; reports any indication of residents' deteriorating physical, emotional, or mental health by noting such cues as uneaten food, deteriorating cleanliness, or conversations that indicate deep unhappiness or loneliness
  • Assists management with resident evaluations to determine individualized care plans and attends resident review team meetings as needed
  • Maintains communication with providers and family members as requested and within the confines of federal and state regulations
  • Responds to resident needs including emergencies, medication administration, and support with activities of daily living
  • Appropriately addresses allegations or sightings of potential abuse/neglect by protecting and reassuring the resident, reporting the issue immediately to management
  • Maintains access to and answers the care base telephone promptly or delegates the task to another team member
  • Keeps work environment clean and tidy
  • Ensures that infection control protocols are followed by team members
  • Maintains adequate supplies and equipment for the unit and records supplies and equipment to be ordered
  • Evaluates work processes and methods and submits recommendations for continuous performance improvement
  • Assumes responsibility for own professional development including meeting all mandated standards and staying abreast of developments within the field
  • Attends scheduled training/education for professional development that includes, at a minimum, trainings required by Williamsburg Landing and by regulatory and accrediting bodies
  • Performs other duties as assigned
  • As appropriate, completes daily skilled notes to document resident observations, changes in condition, response to care, delivery of medications and treatments, and other pertinent information according to departmental standards, policies and procedures; maintains and updates residents' medical records as necessary
  • Rounds on assigned residents at least twice per shift to ensure resident safety and implementation of the plan of care; makes rounds with outgoing and incoming nurse to discuss residents on caseload and observed changes
  • In absence of the Director/Manager and/or Supervisor during weekend or evening/night shifts, responds to employee incidents or injuries to triage/provide first aid and investigate and document events according to Employee Incident Procedure Guide; reports all incidents involving staff injury promptly to DON/Supervisor and Talent Management

QUALIFICATIONS
Must be a Licensed Practical Nurse in the Commonwealth of Virginia; ability to read and write effectively in English; two (2) years of nursing experience in long-term care, acute care, and/or assisted living setting preferred
KNOWLEDGE, SKILLS, AND ABILITIES
Ability to communicate effectively in English, both verbally and in writing, to fulfill job responsibilities; comprehensive knowledge of nursing procedures, theory, and practice with an emphasis on current trends in gerontological nursing; ability to work effectively as a member of a team and to support the development of CNAs; ability to exercise sound professional judgment; ability to respond appropriately in an emergency; ability to lift and carry a minimum of 50 pounds; requires standing and walking for entire shift, safely lifting and transferring residents; must obtain CDP certification within the first year of hire and maintain current CPR certification
*Employees may be required to work overtime when conditions that affect business needs make such scheduling necessary.
