Position Overview
Envision Health Inc. is seeking a compassionate and motivated ACT Registered Nurse (RN) to join our multidisciplinary Assertive Community Treatment (ACT) Team. The ACT RN provides integrated behavioral and physical healthcare services to adults with serious mental illness (SMI), co-occurring disorders, and complex medical needs in community settings.
This position plays a critical role in promoting recovery, wellness, medication adherence, crisis stabilization, and community integration while maintaining compliance with Virginia DBHDS, DMAS, and TMACT standards.
Essential Duties & Responsibilities
- Provide nursing services within the ACT team model in homes, shelters, hospitals, and community settings
- Conduct health assessments, nursing evaluations, and medication education
- Administer and monitor medications as ordered by medical providers
- Coordinate care with psychiatrists, primary care physicians, pharmacies, hospitals, and community providers
- Monitor medication compliance, effectiveness, and side effects
- Assist with medication management, injections, and medication reconciliation
- Provide education regarding chronic medical conditions, wellness, nutrition, and self-care
- Participate in daily ACT team huddles and treatment planning meetings
- Assist with crisis intervention and psychiatric stabilization services
- Support individuals with transitions from hospitals, jails, shelters, or residential placements
- Maintain accurate and timely clinical documentation in the electronic health record (EHR)
- Participate in after-hours on-call rotation as required
- Promote person-centered, recovery-oriented, trauma-informed care
- Ensure compliance with all Medicaid, DBHDS, HIPAA, and organizational requirements
Qualifications
- Active Registered Nurse (RN) license in the Commonwealth of Virginia required
- Minimum of one (1) year experience in behavioral health, psychiatric nursing, community mental health, or substance use treatment preferred
- Knowledge of serious mental illness, psychotropic medications, and co-occurring disorders preferred
- Strong crisis intervention and de-escalation skills
- Ability to work independently and collaboratively within a multidisciplinary team
- Excellent communication and organizational skills
- Valid Virginia drivers license with reliable transportation required
- Ability to travel throughout service areas daily
