• Conduct in-depth psychosocial assessments of patients and their families to identify mental health issues, emotional needs, and available resources.
• Provide individual and family therapy as well as group sessions using proven therapeutic methods such as cognitive-behavioral therapy, solution-focused therapy, and trauma-informed interventions.
• Develop and execute tailored treatment plans that include specific goals and therapeutic interventions, regularly adjusting these plans based on patient progress and feedback.
• Collaborate closely with interdisciplinary teams, including physicians, nurses, and chaplains, to create cohesive, client-centered treatment strategies that enhance overall patient care.
• Ensure compliance with ethical and legal standards by maintaining accurate clinical documentation, treatment records, and progress notes in line with organizational policies.
• Address any health-related questions from members, providing educational resources and fostering a sense of accountability for their health journey.

The ideal candidate will primarily work with patients experiencing serious illnesses who are in hospice care. This population often faces complex emotional and psychological challenges, necessitating a compassionate and skilled approach. Support will also extend to family members as they cope with their loved one's conditions, helping to facilitate conversations about grief, acceptance, and emotional well-being during this sensitive time.

• Master's degree in Social Work from an accredited institution.
• Current, active state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of three (3) years of experience in mental health or outpatient settings, particularly within a hospice or palliative care context is preferred.
• Robust knowledge of evidence-based therapeutic practices and treatment modalities relevant to hospice care.
• Exceptional interpersonal, communication, and assessment skills conducive to establishing a therapeutic rapport with patients and families.

• Experience working with terminally ill patients and their families, emphasizing emotional support and end-of-life issues.
• Proficiency in telehealth practices and electronic health records.
• Working knowledge of local and state regulations regarding hospice care and social work practices.
