• Conduct thorough psychosocial assessments to evaluate clients' challenges, strengths, and support systems. This includes understanding their emotional, social, and financial needs related to their health conditions.
• Implement tailored therapeutic interventions that align with evidence-based practices, including cognitive-behavioral therapy and trauma-informed care, to address complex mental health issues such as grief, depression, and anxiety.
• Develop, revise, and coordinate individualized treatment plans featuring specific goals and therapeutic strategies to address the unique needs of hospice patients and their families.
• Lead and facilitate virtual group therapy sessions and support groups that foster a sense of community and belonging among patients and caregivers.
• Offer crisis intervention and emotional support during critical moments, ensuring that clients and their families have access to compassionate care during times of distress.
• Maintain meticulous documentation of all client interactions and treatment progress in compliance with ethical standards and legal requirements, utilizing our electronic health record (EHR) systems for efficiency.
• Collaborate with an interdisciplinary team, including medical personnel and other mental health professionals, to ensure that all aspects of a client's care are interconnected and comprehensive.

In this role, you will primarily work with patients facing terminal illness and their families, where emotional support, coping strategies, and mental health interventions are essential. Your work will thus significantly influence the quality of life and mental wellness for individuals during their most vulnerable times. Empathetic communication and the ability to create a safe space for clients to express their feelings will be paramount.

• A Master's degree in Social Work from a Council on Social Work Education (CSWE) accredited program.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• Demonstrated understanding of current mental health practices, especially in grief counseling and support for patients in hospice care.
• Exceptional interpersonal skills, including high emotional intelligence and active listening to foster trust with individuals and families.
• Experience with telehealth platforms and electronic record systems is advantageous.

• A minimum of two years of experience in a mental health or outpatient setting, particularly with hospice or palliative care populations.
• Familiarity with evidence-based therapeutic modalities tailored to chronic illness and end-of-life challenges.
• Experience working collaboratively within an interdisciplinary healthcare team.

The salary for this role ranges from USD 80,000 to 100,000 per year, complemented by a competitive sign-on bonus.
