This position is primarily remote, allowing you to perform your duties from the comfort of your home while having the flexibility to engage with clients within the school setting. The typical work hours are Monday through Friday from 8:00 AM to 4:30 PM, with occasional evenings or weekends as needed to meet client demands. • Conduct comprehensive assessments of seniors' psychosocial needs, taking into account emotional, mental, and social factors to create a tailored care plan.
• Collaborate with clients and their families to set realistic, personalized objectives in their care management.
• Provide one-on-one counseling sessions, helping seniors navigate life transitions such as moving into assisted living or coping with the loss of loved ones.
• Act as an advocate for clients, ensuring they have access to necessary community resources and services that cater specifically to their needs.
• Interact and work alongside multidisciplinary teams, including healthcare professionals, nurses, and caregivers, to streamline and enhance client care efforts.
• Regularly monitor and evaluate the stability of clients’ situations through home visits, identifying any safety concerns or quality of life issues.
• Facilitate effective communication among family members to resolve conflicts and clarify roles in care delivery.
• Educate clients and families about various community programs, support groups, and resources that enhance social engagement and support.
• Maintain meticulous records of client interactions, assessments, and care plans within the electronic health records (EHR) system, ensuring compliance with regulations.

Your primary focus will be on supporting seniors, helping them navigate their unique challenges, and advocating for their rights. Working within the school setting, you will also engage with teachers and staff to address any psychosocial issues students may face, ensuring a comprehensive view of health and wellness for both elders and the younger population in the community.

• A Master’s degree in Social Work from an accredited institution.
• Valid state licensure as a Licensed Master Social Worker (LMSW).
• At least two years of professional experience in social work, ideally in a geriatric or community center setting.
• Solid understanding of the principles of social work, aging-related challenges, and community resources available to assist clients.
• Outstanding assessment, counseling, and problem-solving abilities to address complex client needs.
• Exceptional communication skills and the ability to build rapport quickly with clients and families.
• Familiarity with electronic health record systems and proficiency with computer applications.

• Experience working in a multidisciplinary team environment.
• Prior work with senior populations focusing on mental health, emotional support, or case management.
• Exposure to educational settings, collaborating with educators and support staff regarding family dynamics and support systems.
