• Conduct thorough clinical assessments to identify patient needs and develop individualized treatment plans that reflect their goals and preferences.
• Implement evidence-based therapeutic interventions, including counseling and psychotherapy, tailored to patients facing terminal illness.
• Maintain up-to-date and accurate clinical documentation, ensuring compliance with regulatory and organizational standards.
• Collaborate with a diverse team of healthcare professionals to provide comprehensive, coordinated care that addresses both physical and emotional aspects of patient wellbeing.
• Educate and support patients and their families, offering resources and guidance to navigate the complexities of hospice care.
• Participate in team meetings and case reviews to ensure collaborative approach to patient treatment and contribute to ongoing quality improvement initiatives.

In this role, you will work closely with patients in hospice care, many of whom are facing terminal illnesses. Your skills in empathy, active listening, and solution-oriented thinking will be vital as you assist them and their families in dealing with grief, loss, and end-of-life planning. • A Master's degree in Social Work from an accredited institution.
• Valid Licensed Clinical Social Worker (LCSW) license in Georgia.
• Strong knowledge of evidence-based psychotherapeutic interventions, particularly those relevant to end-of-life care.
• Excellent verbal and written communication skills, along with proficient problem-solving abilities.
• Demonstrated ability to work ethically and professionally in a remote therapeutic environment.

• Experience in hospice or palliative care settings is highly desirable.
• Familiarity with solution-focused therapy and grief counseling methods.
• Proficiency in using telehealth platforms and electronic health record systems is a plus.
