Schedule: Monday - Friday; 8:30am - 5:00pm; PRN/as needed

Job Summary and Qualifications

As an ER Discharge Coordinator, you will be responsible for Point of Care (PoC) Patient Bedside scheduling as part of the company’s centralized ER and Urgent Care Follow-up programs. The truest intent of the ER discharge coordinator is a bedside-initiated patient experience, with the objectives of increased contact rate, scheduling immediacy, and improved overall patient experience. You will create and maintain a positive customer experience in a patient-centered, quality-driven environment.  


In this role you will: 


  • Speaks with customers, patients and stakeholders in a pleasant and professional manner; Communicates appropriately and clearly with department leadership, co-workers and customers/patients.  
  • Will schedule patients for follow-up care bedside when appropriate.  
  • Ensures performance and quality standards are met.  
  • Provides superior customer service to internal and external customers and patients; Supports the vision and strategy of the organization, positively promotes the services offered, and elevates issues to leadership as appropriate.  
  • Suggests ideas for positive changes to department policies and procedures.  

Qualifications: 


  • One year of related experience is preferred.
