As a Social Worker at Sunnyview Nursing and Rehabilitation Center, you will play a vital role in providing exceptional care and support to our residents. Our goal is to create a warm and welcoming environment that fosters independence, comfort, and well-being.

About the Social Worker Role:

As a Social Worker, you will be responsible for assessing the needs of our residents, developing and implementing individualized care plans, and ensuring that our residents receive comprehensive and compassionate care. You will work closely with our interdisciplinary team, including physicians, nurses, and therapists, to provide a holistic approach to care. Your expertise in social work will also enable you to connect our residents with community resources and services, helping to promote their independence and quality of life.

Social Worker Responsibilities:

  • Assess the social, emotional, and psychological needs of our residents and develop individualized care plans to address these needs

  • Collaborate with the interdisciplinary team to provide comprehensive care and support to our residents

  • Connect our residents with community resources and services to promote independence and quality of life

  • Provide emotional support and counseling to our residents and their families

  • Develop and maintain relationships with community agencies and organizations to ensure that our residents receive the best possible care

  • Maintain accurate and detailed records of resident assessment, care plan development, and service provision

  • Participate in staff education and training programs to stay current with industry best practices and standards

Social Worker Requirements:

  • Bachelor's degree in Social Work or related field preferred

  • Prior experience in social work in a nursing home preferred

  • Strong communication and interpersonal skills

  • Ability to work effectively in a fast-paced environment

  • Compassionate and empathetic attitude towards residents and their families
