Job description

We are seeking an experienced Primary Care Physician (MD/DO) to provide comprehensive care for frail elderly population. This is a premium position offering competitive rates and a chance to make a meaningful impact in a growing program. Position Highlights:

  • Location: Indiana – Kokomo
  • Schedule: Monday–Friday, 8:00 AM–4:30 PM (No weekends or holidays)
  • Contract: 6-month assignment (likely to extend)
  • Start Date: ASAP
  • Patient Volume: Low initially (2–4/day), growing to 10–12/day
  • EMR: IntusCare

Responsibilities:

  • Deliver high-quality primary care in clinic and home settings
  • Manage acute and chronic conditions
  • Collaborate with interdisciplinary care team (IDT)
  • Provide preventive health services and care coordination
  • Serve as supervising physician for one NP

Requirements:

  • MD or DO from accredited medical school
  • Indiana license and DEA
  • Board Certified in Family Medicine, Internal Medicine, or Emergency Medicine (No BE)
  • BLS certification (or willing to obtain within 2 weeks)
  • Minimum 3–5 years' experience in primary care with frail elderly or long-term care
  • Must meet all federal, state, and local requirements
