Job Overview:
We are currently seeking a compassionate and skilled Home Health Registered Nurse - RN to provide personalized, one-on-one patient care throughout Christiansburg and the surrounding areas. This position is ideal for an RN who enjoys working independently while advocating for patients and helping them achieve the best possible outcomes within the comfort of their home.
As a Home Health Registered Nurse - RN, you will provide home-based care for patients recovering from illness, injury, surgery, or chronic conditions while collaborating with physicians and supervising members of the care team.
Responsibilities of Home Health Registered Nurse - RN:
  • Provide home-based nursing care to patients recovering from illness, injury, or chronic conditions
  • Conduct patient assessments and monitor patient progress
  • Develop and implement individualized care plans
  • Educate patients and families on medications, treatment plans, diet, exercise, and home safety
  • Collaborate with physicians and interdisciplinary care team members
  • Supervise LPNs, CNAs, and aides involved in patient care
  • Document patient progress and report changes in condition to physicians
  • Coordinate discharge planning and ensure patient goals are met
  • Maintain accurate and timely documentation

Qualifications of Home Health Registered Nurse - RN:
  • Graduate of an accredited nursing program
  • Active, unrestricted RN license in the state of Virginia
  • Minimum of 1 year of nursing experience (home health preferred)
  • CPR certification and TB baseline screening required
  • Knowledge of state and federal home health regulations
  • Strong clinical judgment, communication, and critical thinking skills
