Job Summary
The Virtual Care RN will work in close collaboration with a diverse and high functioning patient care delivery team. They will provide real-time clinical support in a centrally located virtual environment. Responsible for partnering closely with bedside nursing staff, providers, and care coordination teams to ensure accurate documentation, patient education, compliance with care protocols, and safe transition of care. The virtual nurse will enhance efficiency, consistency, and patient experience while reducing bedside nurse workload.
oHours are 9:00am - 9:00pm, 7 days per week
oRNs will work 3/12's
oThis is not a remote position

MINIMUM QUALIFICATIONS
Education: Graduate of an accredited School of Professional Nursing; Minimum Associates Degree of Nursing required; Bachelor of Nursing preferred
Licenses/Certifications:
• Current State of Texas license or temporary/compact license to practice professional nursing required
• Certified in Basic Life Support required
• Additional certifications may be required based on discipline and/or nursing unit
Experience / Knowledge / Skills:
• Five (5) years of recent acute care bedside nursing experience required
• Demonstrated excellent critical thinking skills with advanced assessment capabilities
• Effective communication (oral/ written) skills
• Excellent interpersonal skills with ability to demonstrate empathy
• Strong organizational and time management skills with ability to manage multiple priorities of equal importance in a fast-paced environment
• Strong analytical, data management and PC skills
• Demonstrated proficiency with digital health tools
PRINCIPAL ACCOUNTABILITIES
• Collaborate with interdisciplinary team in the delivery of virtual patient care, utilizing the nursing process.
• Provide professional virtual nursing care to patients, with a focus on admission and discharge, patient education, and medication reconciliation.
• Actively facilitate new patient admission; including history collection, medication reconciliation, and required screening questions.
• Utilize evidence-based practice to provide best in class virtual care.
• Follow established guidelines throughout the intake process, including history collection, medication reconciliation, and required screening questions.
• Coordinate and complete discharge teaching via virtual platform, including medications, follow-up appointments, and care instructions.
• Identify barriers to discharge and escalate issues promptly.
• Deliver safe and appropriate virtual care to patients, according to the nursing skills inventory and the age and needs of the patient/population served.
• Participate in workflow improvement initiatives related to virtual nursing.
• Support quality, patient experience, and throughput metrics related to admissions and discharges.
• Utilize virtual technology to maintain visual and verbal connection with patients.
• Coordinate the planning for the delivery of virtual nursing care with an interdisciplinary team and administer appropriate clinical decisions in the delivery of patient care.
• Participate in the coordination of treatment plans; develops/implements discharge and/or transfer plans.
• Collaborate with the healthcare team in the attainment of positive outcomes based on data, research, evidence, and professional standards of care.
• Consider the diverse needs of each patient through the entire continuum of care, providing and ensuring the highest level of service according to established service standards, optimizing the patient experience.
• Adheres to all regulatory and Texas Board of Nursing standards.
• Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
• Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as a resource to less experienced staff.
