Open DoorJOB SUMMARY:

The Case Manager is responsible for providing supportive services to clients and participants of the facility where they are assigned, a supportive services program for homeless adults. Under the direction of the Program Manager (“PM”), the Case Manager works individually with each client in his or her assigned case load, will be available to provide supportive services, as needed, to clients outside his or her case load if the need presents itself. The project will provide safe, secure shelter, meals, and case management on a 24-hour basis for adults experiencing homelessness. The site will incorporate low barrier, trauma-informed care, and harm reduction policies and procedures into their program design and housing operations.

ESSENTIAL FUNCTIONS INCLUDE, but are not limited to the following:

Provides case management services for each client, including but not limited to:

Conducts a needs assessment that includes basic physical and social needs and develops goals and objectives for client.

Properly documents client file.

Makes appropriate referrals and follows up to assure delivery of service.

Helps develop transition plans, including assistance with housing, aftercare services, etc.

Presents cases on a weekly basis for case review and works cooperatively with other members of the program team.

Networks with other social service agencies and other WCA programs.

Confers with the Program Manager on a regular basis to assess client’s participation and revise service plan as necessary.

Compiles statistics as determined by Program Manager.

Conducts life skills and motivation sessions.

Is key contact person for the program in the absence of the Program Manager.

Works effectively with co-workers, customers and others by sharing ideas in a constructive and positive manner; listening to and objectively considering the ideas and suggestions from others; keeping commitments; keeping others informed of work progress, timetables and issues constructively to find mutually acceptable and practical business solutions; addressing others by name, title or other respectful identifier, and respecting the diversity of our work force in actions, words and deeds.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:

Substantial experience in management, program development and staff supervision, preferably in a social service agency.

Experience in the ethics and delivery of social services to homeless and/or low-income people.

Strong writing skills and the ability to write speeches and articles using original and innovative techniques and styles.

Excellent organizational, interpersonal and communication skills.

Ability to make effective and persuasive speeches and presentations on controversial and complex topics to top management, public groups and boards of directors.

Computer proficient (working knowledge of word processing and spreadsheet software: Windows, Word, Excel).

Proficiency in working with mathematical concepts such as probability and statistical inference. Ability to apply concepts of fractions, percentages, ratios and proportions to practical situations.

The ability to manage several projects simultaneously.

Exhibits professional manner when interfacing with professional staff, WCA visitors, contractors, vendors and programs and services providers.

Flexibility in meeting deadlines and adjusting to changing priorities.

Sensitivity to the needs of low income and homeless clients.

REQUIRED EDUCATION/TRAINING AND EXPERIENCE:

B.S./B.A. degree in social services preferred, plus 2 years of relevant social service experience. Equivalent experience in exchange for education considered.

PHYSICAL AND MENTAL REQUIREMENTS AND WORK ENVIRONMENT:

Standing and sitting for long periods of time along with being able to move and walk to various locations; climbing, stooping, kneeling, reaching, pulling, pushing, bending, twisting, and lifting (30-35 lbs.), some driving and stair climbing.

Ability to see, hear, and talk clearly in order to receive instruction and to give instructions as a director.

Ability to operate a computer efficiently. Must be able to operate all types of office equipment.

Work indoors in temperature-controlled environment.

Complex reading and writing skills. Ability to read, proof, and analyze complex documents.

High level concentration and attention to detail for extended periods of time required.

Ability to quickly organize and communicate thoughts and to understand communications from others.

Ability to work independently, with others, around others, have verbal contact with others, face-to-face contact with others, prioritize work and make decisions.

Ability to communicate effectively with individuals in the organization, on the Board, and outside providers and consultant.

Must be fully vaccinated against COVID-19 as defined by CDC.
