Job description

POSITION SUMMARY
We are seeking dedicated and compassionate individuals to join the team at Polara Health, where our mission is to provide accessible, compassionate whole person care that strengthens our communityThe COT Case Manager provides Adult Client outpatient services for the Court Ordered Treatment Program, Clinical Care Services and Case Management Services.
ESSENTIAL RESPONSIBILITIES
• The Court Ordered Treatment (COT) Case Manger is responsible for reviewing COT Renewals in a timely manner, Judicial Reviews every 60 days and backup for Monthly COT reporting for insurance and Arizona Health Care Cost Containment System (AHCCCS) compliance.
• Administrative duties include maintaining spreadsheets, filing Court documents in a timely manner and providing/receiving a Letter of Intent (LOI) for Client transfers to be filed with the Court. Assist and facilitate the Court with Rule 11 evaluations and Justice Court for Clients.
• Instrumental in keeping the Client compliant with medications and Provider appointments. Interacts with Law Enforcement for secure transport to an inpatient setting as recommended by the Inpatient Medical Director.
• Interacts with Inpatient Court Ordered Evaluation (COE) team, Quality Management and Clinical teams.
• Provides care and services for Adults, Legal Guardians, Fiduciaries and Families with multifaceted needs that may include stabilization in living environment, coordination of placements, and support and services when discharging from higher levels of care including the Arizona State Hospital.
• Services may include comprehensive assessment, treatment planning, Assertive Community Treatment (ACT) Services, individual and collaboration with clinical team to determine and guide service provision.
• Provides monthly authorization to insurance carrier for continued stay Behavior Health Residential Facility (BHRF), and Substance Use Disorder (SUD) Program.
• Facilitates inter- Regional Behavioral Health Authority (RHBA) Transfer as needed.
• Provides case management services to COT Clients through assessments, treatment plans, safety plans, Level of Care Utilization System (Locus) and crisis plans.
BACKGROUND
• Confidentiality: This position carries a high level of exposure to confidential information pertaining to clinical and private information.
• Mental Application: Position requires a high level of independent judgement, autonomy and responsibility.
• Responsibility: This position is a Program Lead position with a medium degree of responsibility.
• Contacts/Internal and Public: This position requires a great deal of public, client and collegial contact. It calls for strong public relations skills and problem-solving skills involving diplomacy and sensitivity.
• Special Populations: Substance Abuse, Serious Mental Illness (SMI), COT.
QUALIFICATIONS
• 21 years of age or older; or minimum 18 years old and licensed or certified under A.R.S. Title 32 and providing services within the personnel member's scope of practice; or minimum 18 years old for select administrative positions
• Must be able to provide or obtain an AZDPS Level One Fingerprint Clearance Card
• Must possess and maintain a valid Driver License, current automobile insurance, and a driving record that meets the guidelines and requirements of the organization
  • Bachelors or Master's Degree in Social Work, Psychology or closely related field
  • Four years' experience with SMI population
  • Knowledge of Managed Care preferred
  • Experience working with individuals in crisis situations and/or in need of intensive support services preferred
  • Ability to assess situations and demonstrate critical thinking and problem-solving abilities
  • Understanding of DSM 5
