This job is located in Peoria,IL
Friday Nights - 6:00pm to 6:30am
3rd shift
POSITION SUMMARY: The Digital Health Care Assistant is an integral member of a multidisciplinary virtual care team, supporting critically and chronically ill patients across the Ministry through advanced digital health platforms. Operating within a technology-enabled environment, the assistant facilitates communication, documentation, and coordination of care to ensure timely and accurate information flow between patients, families, and clinical providers. This role encompasses a blend of complex and routine administrative tasks, proactive patient monitoring, and early intervention strategies aimed at improving patient outcomes. Supports continuity of care by managing virtual workflows, engaging in proactive monitoring, and facilitating timely interventions. Through compassionate engagement and technical proficiency, enhances patient outcomes by maintaining consistent communication and ensuring seamless access to healthcare services. This approach helps optimize the quality of life for patients receiving care at home or in virtual intensive care settings.
Qualifications
REQUIRED QUALIFICATIONS:Education: High School Diploma/ GED Experience: 1 year of clerical experience in a healthcare environment. Proficient in EMR and electronic communications
