Minimum Requirements

  • Master's or Doctoral Degree in Physical Therapy from an accredited program. Bachelor's degree acceptable if candidate meets state licensure requirements.
  • Active WV Physical Therapist license or ability to obtain WV licensure prior to start.
  • BLS certification from AHA or ARC preferred. May be required prior to start.
  • Minimum clinical experience providing home health services or outpatient/home care experience preferred.
  • Experience managing caseloads across geographically distributed patient visits and documenting care electronically.
  • Valid driver's license and reliable transportation appropriate for home health travel within Fayette and Nicholas counties.
  • Ability to participate in on-call rotation as the program scales; weekend coverage rotates among therapists.
  • Comfort adhering to agency dress code (navy blue scrubs).
  • Preferred: prior experience with home health documentation systems, productivity tracking, and CLC/GL coding familiarity.
Assignment Details
  • Facility Type: Home Health.
  • Shift Name: Days.
  • Shift Details: Days 0800-1600. (8:00am - 4:30pm noted in client details.)
  • Coverage Areas: Fayette County and Nicholas County, WV.
  • Assignment Type: Travel contract with a minimum 13-week expectation.
  • Weekly Hour Expectation: 32 hours guaranteed per client notes.
  • On-call/Weekend Requirement: Rotating on-call schedule among PT staff as program grows.
  • Floating: Travel between patient homes across the coverage area is required; no floating to other facility sites is specified.
