Job description

The Institute for Human Services (IHS) is Hawaii's most comprehensive human services agency dedicated to ending and preventing homelessness. For over 45 years, we've been at the forefront of creating tailored solutions for those in crisis, offering a continuum of care that includes emergency shelters, housing programs, health services, employment assistance, and outreach.

The APRN-Rx works in partnership with programs internal to IHS including but not limited to the outreach and clinical teams as well as the IHS Health Services Clinics. Functions as one of IHSs primary psychiatric providers of treatment and diagnosis for guests and clients of IHS. Prescribe medications, advise program staff on health/mental health issues, chronic disease, and engage in continuous education of guests/clients to maintain optimum mental health and wellness. Ensure that best practices and evidence-based care achieve positive client outcomes that meet program(s) standards.

Compensation:

Starting from $90/hr.

Essential Functions:

  1. Demonstrate excellent customer service consistent with the standards contained in the Vision, Mission, and Values of the organization.
  2. Diagnose clients of IHS in need of mental health services and work collaboratively with IHS staff to ensure identification of mental health, diagnoses and linkage to access medications/appointments that support medication adherence and success; Provide consultations as needed to manage health, chronic disease and the mental health of guests and clients.

  3. Work collaboratively with program staff to meet the medical, psychiatric and social needs of IHS guests and clients; works proactively with staff in advance of homeless individuals entering IHS shelters to ensure successful placement and retention.

  4. Assist case managers/outreach workers in communicating with primary providers, regarding any healthcare issues including, but not limited to the management of chronic illness, medication management, diagnostic procedures, and the need for health/mental health education and support.
  5. Regularly document and complete required documentation on clients/guests served.

Required Knowledge/Skills/Abilities:

  1. Working knowledge of mental health and medical needs of homeless and/or disenfranchised populations.
  2. Possess excellent interpersonal, problem solving, and multi-tasking skills.
  3. Able to perform psychiatric assessments, develop appropriate formulations, interventions and service plans that assist individuals with complex problems and document accurately in the systems of record.
  4. Current BLS or CPR certification.
  5. Possess personal qualities of integrity, credibility, and commitment to mission.
  6. Flexible and able to multitask; can work within an ambiguous, fast-moving environment.

Required Education and Experience:

  1. Advanced Practice Nursing and Rx license in good standing.
  2. Have at least 2 years of work experience in psychiatric nursing or working in community health.
