Nurse Practitioner Opportunity – P3 @ Home Program
Caring and competent, NPs wanted to provide expert care to our Medicare patients, as well as responding to patients’ medical conditions and history, conducting physical examinations, accurately diagnosing, ordering, and interpreting radiology and lab tests, ordering referrals to specialists, prescribing medications, discussing treatment plans, and appropriately billing and coding. In addition to managing patients and their conditions, the NP will participate in multidisciplinary teams to promote our collaborative approach in caring for patients.

By joining our team, you and your patients have access to our local team of experienced professionals, which includes MDs, NPs, social workers and RNs.
OUR P3 TEAM WILL:
  • Help you understand your patients’ risk and needs.
  • Act as a liaison between you and the Health Plans - immediate help and direct support for you and your patients.
  • Align funding for appropriate care.
  • Grow your practice by attracting new patients.
  • Serve as an extension of your team outside the office to support patients.
  • Utilize tools to manage clinical and financial resources.
  • Offer market competitive compensation, benefits, and provider growth opportunities.
Primary Responsibilities
  • Conduct comprehensive in-home health risk assessment to identify all active and chronic disease conditions as well as determine all physical, mental and social needs present at the time of the visit including an evaluation of past medical history, reviews of systems, medication reconciliation, vital signs, and comprehensive physical exam.
  • Perform evidence-based practice screenings including point of care testing (as appropriate).
  • Identify diagnoses to be used in care management and active medical management in the furtherance of treatment.
  • Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment.
  • Educate members on chronic health conditions, medications, preventative screenings/tests, and lifestyle concerns
  • Communicate findings in your assessment that will be used to inform member’s PCP of potential gaps in care.
  • Identify urgent and emergent situations and intervene appropriately.
  • Identify internal and external referral needs when available.
  • Educate members on topics such as disease process, medication, and compliance.
  • Participate in a rotating On-Call pool, which may include after-hours, weekend and holiday calls and
Qualifications
  • Medical/Master’s Degree and Certification
  • Active NP License or ability to obtain by start date. Licensure must be unencumbered, free of any open/unresolved disciplinary actions including probation or restrictions against privilege to practice.
  • Maintain a controlled substance license along with an unrestricted State DEA license
  • Minimum of 3 years of nursing experience
  • Valid State Driver’s License with a clean DMV record
  • Copy of recent Vehicle Insurance required
  • Ability to complete physical requirements of the job including lifting a 30-pound bag in and out of the car, navigating stairs and a variety of dwelling conditions, and sit, stand and kneel to perform physical assessment.
  • Must have reliable transportation to travel up to 100 miles (with notice, based on business needs) to complete home visit assessments.
  • Training or Interest in Geriatrics welcome but not required.
  • Home Health care or home visit experience preferred.
  • Technically savvy

Assets:
  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or the ability to obtain national certification and/or NP
  • ICU or ER experience
