Conduit Health is transforming one of the most outdated corners of post-acute care: getting essential medical equipment and supplies into patients' homes. We're the first vertically integrated, AI-powered platform uniting ordering, telehealth, prescriptions, insurance, and fulfillment into one experience. We closed a Series A led by Drive Capital in early 2026 and are scaling fast.
The Role
We're hiring a Licensing and Credentialing Manager to own provider licensing and credentialing for our partner telehealth practices. As we grow across states, your job is to make sure every clinician is licensed, credentialed, enrolled, and ready to see patients on time. You'll own the trackers, the deadlines, and the follow-up, and nothing lapses on your watch.
What You'll Do
  • Run end-to-end credentialing and re-credentialing for telehealth clinicians, including CAQH and primary source verification.
  • Manage multi-state licensing for our providers: applications, renewals, and tracking across boards.
  • Own payer enrollment so providers are live with Medicare, Medicaid, and commercial payers before go-live.
  • Maintain audit-ready provider files and monitor all expirables (licenses, DEA, certifications, NPIs).
  • Partner with Clinical Operations and Compliance to keep providers credentialed and compliant as we scale.

What You'll Bring
  • 3+ years in healthcare credentialing, licensing, or provider enrollment.
  • Experience credentialing and licensing providers (MDs, DOs, NPs, PAs, RNs), including CAQH and payer enrollment.
  • Strong organization and attention to detail across high volumes of applications, deadlines, and renewals.
  • Proactive communicator who follows through with providers, boards, and payers.
  • Comfort in a fast-paced, high-growth environment.

Bonus Points
  • Multi-state telehealth credentialing experience.
  • Familiarity with 1099 clinician models.
  • Experience with a headless EMR.
