Essential Functions

  • Management of Existing and New/Potential Clients/Referral Sources
  • Maintains ongoing support and regular communication to existing as well as new/potential clients and referral sources.  Makes contact on a regular and appropriate basis. (Number of contacts (phone/face to face) to be defined:  # of face-to-face contacts per week, # of phone contacts per week and # of new referral sources per month).
  • Serves as the ‘face’ or the representative and the liaison between VIA Health Partners and the client/referral sources and community education in the Education and Resource Manager’s (ERM’s) determined territory.
  • Plans daily/weekly/monthly activities and goals around regular reports and feedback regarding top, medium and low referral sources as well as client concerns, feedback, or other pertinent information.
  • Maintains (and follows up within VIA Health Partners) that patients referred by ERM’s sources are handled with the highest of quality.  Any concerns are addressed by the ERM.
  • Works very closely with the Referral/Intake team and the patient care teams, communicating any pertinent information relative to exceeding the expectations of the client/referral source.
  • Assists in departmental strategies for building stronger relationships with existing clients/referral sources.
  • Provides regular information and in-services/education to clients/referral sources pertinent to Hospice, Palliative Care and end-of-life care in general.
  • Coordinates educational opportunities within the ERM’s region.
  • Analyze data and develop strategy/plan based on needs of the organization.
  • Attends community-based meetings and events (i.e., Chamber of Commerce, senior resource groups, marketing events held at referral sources, community resource meetings, VIA Health Partners organizational and fund raising events) in ERM’s territory to increase referral sources.
  • Competitive environment information.  Aware of all regional communications and provides feedback to VIA Health Partners on the regional approach of other service providers.
  • Participates in contract relationship between facilities and VIA Health Partners.  Acts as a liaison between the facility and the Senior Director of Clinical Services and the Vice President Finance and Operations in contract negotiations. 

May perform other duties as required.

Minimum Qualification

  • A Bachelor’s Degree in a related field is required.
  • A minimum of 2 years of healthcare sales experience within the counties in which VIA HEALTH PARTNERS serves and Healthcare experience is required.  Hospice experience preferred.
  • Proficiency in Microsoft Office is required.
