About the Role
Haven Of Care Home Health is seeking an experienced home health RN to lead our clinical operations. In this role, you'll oversee daily patient care, mentor field staff, and ensure compliant, person-centered care across every episodefrom admission through discharge.
Responsibilities
Oversee all patient care episodes: Start of Care, recertifications, resumptions, transfers, and discharges
Direct OASIS documentation and Plans of Care (485s) for clinical accuracy and compliance
Supervise field RNs, LPNs, and Home Health Aides, including competency evaluations and supervisory visits
Lead QAPI activities, chart audits, and continuous survey readiness
Coordinate care with physicians, discharge planners, and case managers
Support prior authorization and Medicare/Medicaid documentation requirements
Required Qualifications
Active, unrestricted Indiana RN license in good standing
2+ years of home health clinical experience
Working knowledge of OASIS, Medicare Conditions of Participation, and Indiana Medicaid regulations
Strong leadership and communication skills
Valid Indiana driver's license and reliable transportation
Preferred Qualifications
Prior Clinical Manager, Case Manager, or supervisory experience
Familiarity with EVV systems and home health software
Experience with state surveys, accreditation, or QAPI development
