Job description

Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states. 

We are actively seeking Community Integration Counselors in York, PA. The Community Integration Counselor is responsible for assisting individuals with learning physical, intellectual, emotional, and social life skills. The Community Integration Counselor provides individualized services designed to assist participants in managing and overcoming the difficulties confronted when living in the community.

Responsibilities

  • Support and encourage individuals to increase their ability to be as independent as possible
  • Meet with waiver participants on a regular basis either in the individual’s residence, the community, or an office setting
  • Collaborate with the case manager to develop and facilitate programs which meet the social, emotional, physical, and personal needs of the individual
  • Provide ongoing support for community integration efforts
  • Document individual progress toward acquiring community integration
  • Assist in building a support network between the individual and the community
  • Ensure medical consultation forms are completed, by the physician, for each medical appointment attended
  • Administer medications and ensure proper completion of necessary documentation
  • Coordinate all leisure and volunteer opportunities
  • Monitor the health and medical needs of individuals and immediately report any concerns
  • Drive individuals to medical appointments and activities in the community
  • Ensure program documentation and billable records are completed accurately and timely
  • Employees must cooperate with the licensee and department staff in any inspection, inquiry or investigation
  • Additional tasks and responsibilities may be assigned

Minimum Requirements

  • High School Diploma or GED
  • Bachelor’s Degree preferred
  • Valid driver’s license with a satisfactory driving record
  • Minimum 2 years of relevant experience supporting individuals with intellectual and developmental disabilities
  • Ability to establish a comfortable and supportive relationship with individuals receiving supports
  • Ability to exercise good judgment and remain calm in crisis situations
  • Excellent verbal and written communication skills
  • Excellent time management skills

Working Conditions

  • Transport individuals utilizing your own vehicle or company provided vehicles
  • Schedule may change due to business needs and may include evening and weekend hours
  • May be required to be on-call in cooperation with other management staff
  • Frequent lifting, stretching, and other physical exertion may be required
  • May be required to lift or move 25+ pounds
  • May be exposed to various medical conditions and communicable diseases
