The Family Medicine Physician or Advanced Practice Provider delivers a full range of primary care services to individuals across all ages in a Federally Qualified Health Center (FQHC) setting, including care provided through a mobile health unit. The clinician supports patients with preventive services, ongoing management of longterm conditions, and care for immediate health needs. This role works closely with an interdisciplinary care team and contributes to the FQHC mission of providing highquality care that respects each patient’s background and needs.

Education & Licensure

  • MD, DO, or Advanced Practice Provider degree (NP/PA) from an accredited program

  • Active or eligible for active Georgia medical or APP licensure

  • Provide a full range of primary care services, including wellness visits, care for immediate health needs, and ongoing management of longterm conditions for patients of all ages.

  • Deliver care in a mobile health unit and, when needed, at fixed clinic locations.

  • Perform patient assessments, diagnoses, treatment planning, and followup care.

  • Offer patient education focused on wellness, treatment plans, and selfmanagement.

  • Document care accurately in the electronic health record in accordance with FQHC, regulatory, and payer requirements.

  • Collaborate with nurses, medical assistants, patient registration staff, care coordinators, and community health workers.

  • Maintain continuity of care through appropriate referrals and coordination with specialty and community resources.

  • Participate in team meetings, case discussions, and careplanning activities.

  • May serve as a clinical resource or mentor to advanced practice providers and clinical staff, depending on program needs.
