Join Our Team as a Client Care Coordinator!
Are you ready to make a difference in the lives of others? We are seeking driven individuals that are passionate about client care for the role of Care Coordinator to join our dynamic home care agency. In this role, you will have the opportunity to manage a caseload of assigned clients, ensuring they receive the highest quality of care and support.
This role is a part-time role with the option to be expanded into a full-time position. Working hours will be Monday - Friday between 8am - 5pm with an occasional Saturday Staff Meeting and on-call rotation.
Qualifications:
- High School Diploma or GED required
- Experience in home care, care coordination or a related field preferred.
- Leadership Mindset, Strong organizational skills and attention to detail.
- Excellent communication and interpersonal skills.
- High level problem solving and critical thinking skills
- CNA or Caregiver experience in home care a plus!
What You'll Do:
- Coordinate Care: You'll be the heart of our client care operations, overseeing and coordinating personalized care plans to meet the unique needs of each individual.
- Customer Service: Ensure continuity and reliability of care. Making sure that clients are receiving services as assigned and are happy with services.
- Community Outreach: Work along side marketing and intake to share services with unreached populations.
