Job description

Home Care Liaison

Home Care | New York City, NY

Full Spectrum Search Group is a full-service healthcare recruiting agency specializing in post-acute leadership and healthcare administration, serving skilled nursing, senior living, home health, hospice, home care, and the providers and technologies that support the post-acute space. We are conducting a search for a growing private duty home care organization seeking a Home Care Liaison to drive referral growth throughout New York City and Nassau County.

If you've built your career developing referral relationships, consistently generating private-pay business, and want the opportunity to make a measurable impact in one of the country's largest home care markets, this is the type of opportunity that rarely becomes available.

Four Uniques

  • High-performing liaisons have the opportunity to earn significant commission in addition to a competitive base salary.

  • The organization successfully built its flagship New Jersey operation from startup to millions in annual revenue and is now investing heavily in accelerating growth throughout New York.

  • Leadership is looking for someone who wants to own a territory—not simply maintain existing referrals. You'll have the autonomy to build your network and directly influence market growth.

  • The company also holds a skilled in-home nursing license, creating future growth opportunities beyond traditional private-duty home care services.

You'll spend the majority of your time in the field developing relationships with hospitals, assisted living communities, independent living communities, physicians, case managers, discharge planners, elder law attorneys, and other referral partners. This is an opportunity to join an established organization with strong operational leadership while helping build the next phase of growth in one of its highest-priority markets.

Key Responsibilities

  • Develop and maintain referral relationships throughout New York City and Nassau County.

  • Generate new private-pay home care referrals through hospitals, assisted living communities, senior living providers, physicians, case managers, and community partners.

  • Execute consistent face-to-face sales calls and networking activities within the assigned territory.

  • Represent the organization at community events and professional networking opportunities.

  • Collaborate with clinical and operational teams to ensure an exceptional client experience from referral through admission.

  • Consistently achieve census and revenue growth goals.

Qualifications

  • Proven success in healthcare business development or outside sales.

  • Experience in private-duty home care, home health, hospice, senior living, assisted living, or another closely related post-acute setting.

  • Existing referral relationships within the New York City market are highly preferred.

  • Demonstrated hunter mentality with a track record of generating new business.

  • Strong communication, presentation, and relationship-building skills.

  • Ability to work independently while managing a large geographic territory.

  • Candidates with successful senior living sales experience are encouraged to apply.

Compensation

  • Competitive base salary

  • Commission plan

  • Comprehensive benefits package
