We have an exciting opportunity for a full-time, Mental Health Counselor to join our Gracepoint partial hospitalization program. The Mental Health Counselor for the PHP program provides direct support and therapeutic services within a structured outpatient program serving individuals with mental health needs who do not require inpatient care. This role involves group-based interventions, supports individualized treatment goals and contributes to a recovery oriented program environment. The counselor plays a key role in maintaining program flow, therapeutic structure and client engagement. Demonstrates an understanding of, and embraces, the mission statement of Christian Health.

Competencies:
  • Lead or co-facilitate structured therapeutic, psychoeducational and skill building groups.
  • Support a safe, structured and recovery-oriented environment that provides client engagement and participation.
  • Collaborate closely with the interdisciplinary team and participate in treatment team meetings and intake evaluations.
  • Perform case management duties including coordination of care, discharge preparation and community resource linkage and referrals.
  • Monitor client presentation, behavior and participation; communicate concerns to clinical staff.
  • Support intake, program orientation and transition processes for new and discharging clients.
  • Maintain accurate and timely clinical documentation in compliance with program and regulatory standards.
  • Reinforce therapeutic interventions under clinical supervision.
  • Provide brief supportive check-ins to reinforce coping skills and treatment goals.
  • Support family involvement as appropriate to coordinate care.
  • Performs other related duties as required. 

Experience: Working in a mental health setting or related work experience.

Schedule: 8am-4pm, Monday - Friday.

Education: Bachelor's degree in social work, psychology, counseling or related field.

QUALIFICATIONS/REQUIREMENTS

Demonstrates compliance with the CH ASPIRE Standards of Performance.

Qualifications:

  • Must possess baseline social work skills in listening, exploring, and communicating responses to patients and families.
  • Must have knowledge of Screening, Assessments, and Discharge Planning as indicated by the ability to seek resources and make appropriate referrals.
  • Must be able to legibly record Assessments, Social Histories, and Contact Summaries in the Case Record.
  • Must report progress and problems during supervision with the Director of Social Work and actively participate in Interdisciplinary Treatment Team Meetings. Must communicate as liaison with RRPH Staffand Community Agencies.
