Key Responsibilities
  • Conduct thorough evaluations and assessments of clients to identify their emotional, psychological, and social needs.
  • Engage actively in the development and implementation of Individualized Family Service Plans (IFSPs), ensuring they are tailored to meet the needs of each patient and their family.
  • Monitor and evaluate the outcomes of care, adjusting plans as necessary to ensure the best possible results for families.
  • Provide emotional support and counseling to clients and their families, helping them navigate the complexities of hospice care and their unique situations.
  • Compliance with all Federal and State regulation standards, safeguarding the health and safety of both staff and clients.
  • Develop strong rapport with clients, families, and team members to foster a collaborative and supportive environment.
  • Utilize evidence-based practices for joint planning with families and implement strategies that adhere to Natural Learning Environment Practices.
  • Educate families on child development and available resources, providing essential skills and support to promote healthy growth and adaptation strategies.
  • Conduct regular reviews and updates on IFSPs based on child development status, family priorities, and emerging needs.
Patient Population / Client Focus

In this role, you will primarily work with children diagnosed with disabilities or developmental delays. Your support will extend to their families, empowering them with the tools and knowledge necessary to enhance their child's developmental outcomes. As a member of our compassionate team, you will ensure that each family's unique needs and goals are prioritized, contributing to a holistic approach to care.

Qualifications
  • Master's Degree in Social Work or a Doctorate of Social Work.
  • Valid licensure as a Clinical Social Worker (LCSW) issued by the Department of Public Health.
