We are currently seeking a Pool Medical Records Coder position with a weekend requirement of at least one to two weekends a month. This position will be located at our Milwaukee-based business office and work from home after orientation.The Medical Records Coder is vital to the success of Horizon Home Care and Hospice. This role is essential to not only helping delivery of medical care, but also essential in the billing process to ensure optimal reimbursement.

This individual must be able to represent Horizon with impeccable professionalism and accuracy when interacting with physicians, clinicians and staff.Must be a pro at staying on top of deadlines and follow through with minimal supervision. We need a true team player able to collaborate effectively with multiple departments.

Essential Functions:

  • Assigns and sequences ICD-10-CM codes as appropriate and abstracts pertinent information from patient records, for all admissions to Home Care and Hospice, new and post hospital referrals in an accurately and timely manner.
  • Serves as an agency resource regarding ICD-10-CM coding issues to ensure optimal reimbursement of all cases in compliance with CMS policies and procedures and Official Coding Guidelines

  • High School Diploma or equivalent required. Formal coding education or CCA/CCS required.
  • 2-3 years of medical records coding experience using ICD-10 preferred.
  • HCS-D preferred and Home Care Home Base EMR Knowledge preferred
  • Working knowledge of healthcare computer system and computer assisted coding (CAC)
  • Current driver's license for the State or Wisconsin and proof of current vehicle insurance
