RN Case Manager

Hybrid Role | 2 Days Remote / 3 Days In the Field or Office
Monday–Friday, 8:00 AM–4:00 PM
No nights, weekends, or on-call

Looking for a nursing role with better balance and more flexibility?

We're seeking an experienced RN Case Manager to support seniors with complex health needs in a community-based setting. You'll be working closely with patients in their homes and coordinating care with providers and care teams to help seniors stay safe, supported, and independent at home.

This is a hybrid role with two remote days each week, plus field and office work the rest of the time—so you get a nice mix of independence, patient interaction, and work-from-home flexibility.

What you'll be doing:

  • Completing in-home assessments and developing care plans
  • Coordinating services with providers and community partners
  • Managing a panel of patients and supporting ongoing care needs
  • Helping seniors and families understand and navigate care options

What we're looking for:

  • Active MA RN license
  • 3+ years of nursing experience (home care or case management a plus)
  • Strong communication and organization skills
  • Reliable transportation for local travel
