Position Title: Travel Registered Nurse (RN) - Case Management
Location: Portland, OR
Department/Unit: Case Management
Shift: Variable Shift (7:30 AM to 5:00 PM, 36 to 40 hours per week)
Contract Duration: 13 Weeks
Salary Range: $50 - $60 per hour
Job Summary:
We are seeking a dedicated and experienced Travel Registered Nurse (RN) specializing in Case Management to join our team in Portland, OR. The ideal candidate will have a strong background in nursing and case management, with a focus on providing high-quality patient care. This role requires EPIC training, and candidates should be prepared to work collaboratively with a multidisciplinary team.
Key Responsibilities:

  • Assess patient needs and develop individualized care plans to ensure optimal patient outcomes.
  • Coordinate care across the continuum, facilitating communication between patients, families, and healthcare providers.
  • Monitor patient progress, addressing any barriers to care and adjusting plans as needed.
  • Utilize EPIC software for documentation and care coordination; ensure compliance with regulatory standards.
