Experienced RN Case Manager
Schedule
The shift for this position is from 8:00 AM to 5:00 PM, Monday through Friday, with a start time ranging from 8:00 AM to 9:00 AM. The total work hours will be approximately 8.5 hours per day.
Job Description
We are seeking an experienced RN Case Manager to provide high-quality patient care through effective case management practices. The ideal candidate will possess a solid background in acute care and demonstrate the ability to coordinate and manage patient care comprehensively.
Responsibilities
  • Conduct patient assessments and develop individualized care plans.
  • Collaborate with the healthcare team to ensure care delivery aligns with best practices.
  • Monitor patient progress and adjust care plans as necessary.
  • Support patients and their families through education and community resource navigation.
Required Experience / Certifications / Licensure
  • 2+ years of acute care Case Manager experience required.
  • Active PA RN license required.
  • Bachelor of Science in Nursing (BSN) is required.
Assignment Duration
This is a contract position with guaranteed hours of 40 hours per week for a total of 88 weeks.
