Job description

Director Case ManagementLocation: Charleston, SC

The Facility Case Management Director oversees and coordinates departmental activities, ensuring staff compliance with organizational policies and external regulations. This role leads the integration of patient care and discharge planning processes with hospital departments and external agencies to ensure continuity of care and optimal resource utilization. The Director is responsible for daily operations, achieving outcomes, and engaging interdisciplinary teams and external stakeholders.

Responsibilities:
  • Direct and evaluate department operations, including staffing, technology use, onboarding, and staff competencies to achieve performance and quality objectives.
  • Allocate resources to meet productivity and quality goals.
  • Oversee core case management functions and practices.
  • Ensure adherence to care coordination and discharge planning processes, including documentation.
  • Collaborate with Facility CFO and Division CM Leader to achieve goals.
  • Assess and improve department performance through process evaluation, data analysis, and performance improvement activities.
  • Ensure compliance with policies and standard operating procedures.
  • Establish working relationships with key stakeholders.
  • Practice according to the "Code of Conduct" and "Mission and Value Statement."
  • Perform other duties as assigned.
Qualifications:
  • Bachelor's degree required; Master's degree in Nursing, Health Administration, or Business Administration preferred.
  • Either Registered Nurse (RN) licensure or Licensed Clinical Social Worker (LCSW) required.
  • 5+ years of clinical experience.
  • 3+ years of experience in acute care Hospital Case Management.
  • 2+ years of experience in case management leadership.
