As a Care Partner in our Terrace Club neighborhood, you provide personalized, relationship-based care to a consistent group of residents, helping create meaningful and successful days.
What You'll Do:
โ€ข Provide hands-on care and emotional support based on each resident's Individualized Service Plan (ISP)
โ€ข Partner with residents through prompting, cueing, and reminders
โ€ข Observe, document, and report changes in condition
โ€ข Support daily programs and meaningful engagement activities
โ€ข Assist with dining, housekeeping, and laundry services
โ€ข Promote safety, dignity, and Resident Rights at all times
โ€ข Communicate regularly with families and care leadership
โ€ข Follow infection control, OSHA, and safety standards
You serve as a trusted advocate and consistent presence, building strong relationships that enhance quality of life.
What We're Looking For:
โ€ข High School Diploma/GED; age 18+
โ€ข Experience working with seniors preferred
โ€ข Compassionate, patient, and dependable
โ€ข Strong communication and organizational skills
โ€ข Ability to remain calm and make sound decisions
โ€ข CNA/HHA certification (as required by state, NJ required)
