Job description


ChildServe is seeking an In-Home Medical Day Care Provider to provide personal care and supervision for an individual with special health care needs in their home.  

 

Schedule Various hours between 9am – 5pm M-F

 

How You’ll Make an Impact

  • Provide direct care and assist individuals with daily living and behavioral needs in their home
  • Implement care plans and provide ongoing feedback regarding additional or changing care plan needs
  • Perform duties related to the tracking and upkeep of participant’s personal belongings
  • Responsible for timely and accurate communication regarding services with families, staff, and supervisors
  • Transport individuals to outings in the community

What You’ll Need:

  • Must be at least 18 years old to apply
  • Experience working with or knowledge of individuals with special health care needs preferred
  • Provide your own transportation
  • Ability to electronically document services provided
  • Ability to communicate effectively in reading, writing, and speaking the English language
  • Ability to lift up to 35 lbs. be mobile within the assigned work area, and frequently bend, lift, reach, and stand
